Passport Services
The Winona State University Study Abroad Office is an official U.S. Department of State Passport Acceptance Facility.
We accept new passport applications only. Review the application requirements to see if you qualify.
If you need to renew, replace, correct, or report a missing passport, consult the U.S. Department of State information for those who already have a passport.
All required documents must be present at the time of appointment, or your appointment will be canceled and rescheduled.

For a new adult passport (age 16 and older), you will need the following documentation:
- Proof of Citizenship: Original or certified copy of Birth Certificate, previous passport, Certificate of Naturalization, Consular Report of Birth Abroad
- Proof of Identity: State issued driver’s license/identification card
- Photos: One 2×2 passport photo meeting the US Department of State requirements
- Application Fee: Check or money order payable to the US Department of State
- Execution Fee: Check or money order payable to Winona State University Study Abroad
- DS-11 Application: Filled out in black ink only and printed single-sided or pick up a form in the Study Abroad office, Maxwell 105
For a new passport for a child aged 16-17 years, you will need the following documentation:
- Proof of Citizenship: Original or certified copy of Birth Certificate, previous passport, Certificate of Naturalization, Consular Report of Birth Abroad
- Proof of Identity: State issued driver’s license/identification card
- Photos: One 2×2 passport photo meeting the US Department of State requirements
- Application Fee: Check or money order payable to the US Department of State
- Execution Fee: Check or money order payable to Winona State University Study Abroad
- DS-11 Application: Filled out in black ink only and printed single-sided or pick up a form in the Study Abroad office, Maxwell 105
- Proof of parental awareness: One parent must accompany the applicant or provide a signed statement of consent from one parent with a front and back photocopy of parent’s ID
For a new minor passport (age 15 or younger), you will need the following documentation:
- Proof of Citizenship: Original or certified copy of Birth Certificate, previous passport, Certificate of Naturalization, Consular Report of Birth Abroad
- Proof of Identity: State issued driver’s license/identification card
- Photos: One 2×2 passport photo meeting the US Department of State requirements
- Application Fee: Check or money order payable to the US Department of State
- Execution Fee: Check or money order payable to Winona State University Study Abroad
- DS-11 Application: Filled out in black ink only and printed single-sided or pick up a form in the Study Abroad office, Maxwell 105
- Proof of parental consent: Both parents and the minor applicant must be present for us to accept the application. If one or both parents cannot be present, appropriate documentation must be provided.
Two separate payments are required:
- Application fee: Check or money order only (no cash or credit/debit cards), made payable to the U.S. Department of State
- Execution fee: Check or money order only (no cash or credit/debit cards), made payable to Winona State University Study Abroad

Yes. The Passport Agency will send your original proof of citizenship along with any other original documentation back to you in a separate mailing.
No. Every applicant must apply for a new adult or minor passport in person, including minor/infant applicants.
No. The passport will be mailed to the mailing address indicated on the application.
