Immunization Requirements

All students are required to complete a WSU Student Immunization Record. Those students born on or before Dec. 31, 1956 are required to complete the form, but are not required to supply immunization dates.

All others (except those with a physician signed medical or notarized conscientious exemption) are required to have the following immunizations:

  • Measles, Mumps, and Rubella (MMR): one dose is required after your 1st birthday
  • Diphtheria-Tetanus (tD): Must be within the last 10 years

All public and private colleges and universities must provide information to all first-time enrollees on the transmission, treatment, and prevention of hepatitis A, B, and C, as well as on the risk of meningococcal disease and the availability of meningococcal vaccine.

This information can be obtained from the Minnesota Department of Health and WSU Health & Wellness Services Forms.

The American College Health Association (ACHA) also recommends the following immunizations:

  • Hepatitis B—series of 3 vaccines
  • 2nd dose of Measles, Mumps, and Rubella (MMR) 
  • Meningococcal Vaccine     

*International students and those students returning to the U.S. from countries where Tuberculosis(TB) is prevalent are strongly encouraged to have a Mantoux(TB) skin test administered upon registration.

*Information about immunizations for international travel can be obtained from the Centers for Disease Control and Prevention (CDC).

 

COVID-19 Vaccines 

COVID-19 vaccines are recommended and strongly encouraged for all WSU students and employees. Health Services has vaccines and booster shots available to all WSU personnel. COVID-19 vaccines are required for some on campus. Check out the university's COVID-19 information page for more information on vaccine requirements.

COVID-19 vaccines are free of charge. Please bring your insurance information, student ID and your vaccine card (if receiving second or third dose) to your appointment.

Your vaccination record (sometimes called your immunization record) provides a history of all the vaccines you received as a child and adult.

This record may be required for certain jobs, travel abroad, or school registration.
